The one on the left keeps them stacked, so if you had, let's say, 3 Thornberries, they would all be taken up by one block in your Inventory, with a little circle on it showing the amount you have. If you have the right button toggled, there would be 3 separate blocks for each Thornberry, as they are all unstacked. It's because if you buy say 10 of a gram/cap, maybe 2 of them will have the bonus item - if you're lucky. When it stacks, you have to actively remove the top one from the inventory before you can check the next one. It's a pain in the rear.
