The TDMDGPOP avatar is completely random, there is no set procedure to unlock it. There is no set amount of days that you need to visit it, nor any certain equipment required. How quickly this avatar can be obtained differs on the odds of the player (same thing goes with the Snowager avatar, both avatars can be a pain to get and may take quite a while to obtain).

Everyone knows that Dr. Mario World has turned practically all kinds of Mario-verse characters into doctors, they even turned a Goomba Tower into a doctor. No one said a Meepit couldn't have a turn to join in on the doctor fun.
I made this myself since there weren't any images of a Meepit in a doctor suit. Apologies for any flaws.Spoiler
