Custom User Lookup Request

I would like the style to be similar to my current userlookup, meaning that I want the separate boxes rather than having it all connected. I don't need a custom shield, but if you want that's cool too=]

I want it to be color schemed with the colors in the photo, and would like a border around the seperate boxes. Other than that, it's up to you=D

Hmm at first I thought you were making them too. Perhaps edit your first post and make it a bit more outstanding that you want a userlookup, not making them.
